About Us

St. Paul’s Program of All-Inclusive Care for the Elderly (PACE) is a managed healthcare plan exclusively for seniors. As part of St. Paul’s PACE, you receive primary medical care, medication management, physical therapy, specialty services, home care, social work assistance, and transportation to and from appointments. With St. Paul’s PACE, you can remain living in the comfort of your home and get the medical care you need in a safe environment.

St. Paul’s PACE is specifically designed for seniors 55 years and older who have chronic medical conditions and are struggling to live at home independently.
