Under the direction of the Ambulatory Care Manager, the Ambulatory Care Supervisor oversees the daily clinical operations, and patient care activities of assigned care teams. Within the supervisor role, will carefully balance direct patient care and supervisory duties to ensure the highest quality service and care is provided. Responsible for the delivery of patient-centered services in accordance with the San Ysidro Health Model of Care designed to deliver safe, high-quality, cost-effective care, and improve health outcomes by meeting the population health management needs of the communities served.
Essential Functions of the Job:
- Supervise and coordinate daily team-based functions according to the Patient Centered Medical Home care model. Lead care teams in the achievement of organizational goals for health outcomes improvement, patient safety, satisfaction, and clinical quality goals.
- Follow evidence-based guidelines to assist in the development of department protocols, policies and procedures to optimize clinical outcomes across the continuum of care.
- Implement and monitor approved medical management protocols and pathways to optimize clinical outcomes.
- Select, train, orient, supervise and develop nurses and medical assistants. Evaluate clinical competencies on hire, annually, and as needed and complete performance reviews in collaboration with Clinical Services Manager.
- Maintain optimal staffing schedules and provide direct patient care as needed to support clinic operations.
- Supervise assigned care team staff, oversee care team operations, and develop teams that meet the growing needs of the communities served. Ensure the staff provides the highest quality of care which complies with the Nurse Practice Act, TJC, federal/state/local, and health plan requirements.
- Maintain continuous audit readiness of staff and clinical spaces by being a visible leadership example consistently making rounds in patient care areas and ensuring daily clinical compliance with local/state/federal guidelines and requirements.
- Utilize effective problem-solving techniques and collaborate with Clinical Services Manager to provide disciplinary action as needed to improve employee relations.
- Ensure clinical preparedness to meet patient care needs by overseeing a number of initiatives and programs such as infection control, medication, immunization, and administration compliance.
- Serve as the nursing representative at site Continuous Quality Improvement meetings, and organizational clinical quality meetings assuming accountability for identifying, coordinating, and implementing improvements in the nursing and medical assistant workflow that impact overall patient care workflow improvements.
