• (Registered LPHA) Mental Health Case Manager

    The duties listed below are intended only as illustrations of the various types of work that may be performed.  The omission of specific statements of duties does not exclude them from the position if the work is similar, related or a logical assignment to this class.

    • Facilitate a range of trauma-informed group services with residents who have experienced various traumas with serious illnesses, addictions, disabilities, pregnancy, sexual/ mental /physical abuse, or neglect. Provide therapeutic, case management, and crisis intervention services to residents and families. Ability to work with a diverse cultural population and exercise independent judgment. Provide crisis intervention during working hours and as needed after work hours.   
    • Communicate with other involved parties and agencies on a regular basis, including other resources available to residents. Analyzes and provides advocacy and services, such as needed emergency medical care, or special needs programming Support AOD counselors with co-occurring client caseloads, training in co-occurring services, appropriate assessments and referrals Ensure flexibility of scheduling and crisis intervention services are provided to meet the needs of MAACs’ clients.   
    • Maintain client case files in accordance with contractual obligations and agency standards to include electronic medical records in database. Assist with implementing contractual and programmatic components. Complete all required case file documentation in a timely and routine fashion. Ensure proper documentation as required by DHCS, Medi-Cal, the United States Court System, and San Diego County.   
    • Participate in team meetings, trainings, individual supervision, and clinical group supervision. Participate in agency required trainings, staff meetings, quality assurance, and related activities. Attends trainings required to meet Medi-Cal standards to include: ASAM trainings, Motivational Interviewing, Evidence-Based Practice, and 5 hours of training annually in substance abuse disorders.   
    • Retain Associate Marriage & Family Therapist (AMFT) or Associate Social Worker (ASW) intern number or Marriage and Family Therapist (MFT)/Licensed Clinical Social Worker (LCSW) license with the California Board of Behavioral Sciences Adhere to agency and contract client confidentiality, HIPAA, and mandated reporting policies. Other duties as assigned by the Program Manager.    

    QUALIFICATIONS AND SKILLS

    Knowledge of and Ability to:

    • Able to work with potentially homeless, justice involved, CWS involved, and mentally unstable.
    • Able to work in a very diverse setting with people of all backgrounds, culture, and sexual orientation.
    • Experience in the areas of group, individual, and family counseling.  
    • Experience in the areas of outreach, domestic violence, community education and development, advocacy, and case management.   

    Physical Qualifications:

    • Experience working in a residential setting.
    • Must be willing to develop or possess working knowledge of trauma-informed care and dual diagnosis treatment.
    • Experience working in trauma and dual diagnosis programs preferred.
    • Motivated to take on additional community involvement projects and solve problems.
    • Comfortable in a fast-paced environment with multiple cases.
    • Empathetic and supportive with mentorship and leadership skills.
    • Flexible this is 24 hours – 7 days a week program. (Available to work evenings, overnight, weekends and holidays)   

    EDUCATION/EXPERIENCE/CERTIFICATION

    • Minimum of one year working as a mental health clinician with youth and families.
    • Licensed or Registered Intern with a M.A. or M.S.W. in social work, psychology or related field required.
    • Related work experience dealing with justice system, substance abuse and/or immigrant populations.
    • Bilingual Spanish-English preferred
    • First Aid and CPR Certified preferred.
    • Supervision hours provided by a Licensed Clinical Social Worker (LCSW)
